• please review Part VI, Section 6.6 (Drugfree Workplace Act) carefully and mark the <br /> statement with an 'XI as it relates to your entity. <br /> Your entity's ability to incur costs against the Grant as of the beginning date set forth in the <br /> Notice of Grant Award is contingent upon your entity's acceptance of the terms and conditions as <br /> set forth in the attached Agreement. The Department may elect not to execute the Agreement if <br /> any of the terms are changed without the Department's prior approval, and the Grantee will be <br /> responsible for any costs already incurred. <br /> The Department may also elect not to execute the Grant Agreement if it determines that it is not <br /> in the best interest of the State of Illinois to do so. Written notification of such a determination <br /> and the basis therefore will be provided to the addressee of this letter in such event. <br /> IMPORTANT INSTRUCTIONS: Please return ONLY the signed page 3 of the Agreement and the <br /> first page of Part VI (containing Section 6.6 . Drugfree Workplace Act), as well as the signed page 4 <br /> if naming anyone else as Authorized Designee(s), to your DCEO Contact indicated within Part IV, <br /> Section 4.15 of the Agreement. Please direct any questions concerning the Agreement to the <br /> individual identified in that Section. <br /> ADDITIONALLY, the forms and specific instructions to be utilized in preparing reports <br /> referenced by Part III, Section 3.2 of the Agreement, and identified in the separate Reporting <br /> Schedule within an eventual Welcome Package you will receive along with the executed Grant <br /> Agreement, will be provided to your organization as soon as possible. Reports will not be <br /> accepted unless these forms are used. <br /> ***IMPORTANT: IF YOUR ORGANIZATION HAS RECEIVED PRIOR DCEO GRANTS AND ALL REQUIRED REPORTS, <br /> AND/OR OTHER OBLIGATIONS, HAVE NOT BEEN SUBMITTED AND APPROVED BY THE DEPARTMENT, THERE MAY <br /> BE A DELAY IN THE RELEASE OF FUNDS FOR TH/SGRANT. <br /> You will be provided with a fully executed copy of the Agreement after it has been signed by the <br /> Department's Director. <br /> Sincerely, <br /> 9kat �V� - <br /> Brooke Doggett <br /> DCEO Grant Manager <br /> Enclosure(s) <br />
