d ' <br /> under the grant shall remain the property of the Grantee; however,the Department may use <br /> any or all Grant Data developed as part of the project without restriction and without <br /> compensation to the Grantee. In the event that the Grantee obtains a patent,copyright,or <br /> otherwise restricts access to Grant Data,the Department shall have a royalty-free license to <br /> use the patented invention or copyrighted material for the Department's own purposes. <br /> The Grantee will not publish or permit any other person to publish any report,study,or <br /> other document produced by the Grantee as a deliverable under this grant without the <br /> Department's advance approval. The Grantee shall acknowledge the sponsorship of the <br /> Department in the project in any such publication,or shall accede to any request by the <br /> Department that appropriate disclaimer language be incorporated into the publication. <br /> 6. OUARTERLY REPORTS: The Grantee is required to file quarterly status reports on the <br /> grant project describing the progress of the program,project, or use and the expenditure of <br /> the grant funds related thereto,if the grant amount is over$25,000.00. In accordance with <br /> the Illinois Grant Funds Recovery Act, 30 ILCS 705/1,the Department may withhold or <br /> suspend the distribution of grant funds under this Agreement for failure to file required <br /> reports. <br /> 7. FINAL REPORT: The Grantee shall also submit two(2)complete copies of a Final <br /> Report detailing the methods used to fulfill this.grant and documenting actual expenditures <br /> and match share in completion of the project. -The Grantee must submit this Final Report <br /> within 45 days after the completion of this project or within 45 days after the expiration of <br /> this grant agreement to the Division of Grant Administration at the address identified in <br /> Paragraph 10 herein. If the Final Report is not delivered within this time allowance,the <br /> grant funds remaining for this project may be forfeited and any funds paid out to the <br /> Grantee as advance or interim payments will be returned to the Department. Failure to. <br /> provide a Final Report may result in actions to recover grant funds in accordance with the <br /> Illinois Grant Funds Recovery Act, 30 ILCS 705/1. <br /> ARTICLE 2: STANDARD TERMS AND CONDITIONS <br /> 8. SUBGRANTEE DISCLOSURE: The Grantee shall disclose the use of any subgrantee to <br /> fulfill obligations under this grant by providing the subgrantee's name, address, and <br /> telephone number, a description of the services to be rendered, and the amount to be paid <br /> the subgrantee as part of the project,by completing Attachment B,if applicable. All <br /> subgrantees,if any,shall be listed on Attachment B prior to the completion of the project. <br /> 9. PERSONNEL: All technical,clerical and other personnel necessary for the effective <br /> performance of this grant shall be employed or contracted by the Grantee, and shall in all <br /> respects be subject to the rules and regulations of the Grantee. The Grantee,its employees, <br /> officers,directors or subgrantees shall not be considered to be agents or employees of the <br /> Department or the State of Illinois. The Grantee;its employees,officers, directors or <br /> subgrantees shall be considered to be independent contractors. <br /> 10. NOTICES: Any notices by the Grantee or Department pertaining to this Agreement shall <br /> OVGrantee initials <br />
