GENERAL CONDITIONS , <br /> GC-1 CERTIFICATE OF INSURANCE (Continued) <br /> Certificate: <br /> Upon request the contractor shall file with the City certified copies of all insurance <br /> policies and all accompanying endorsements. The contractor shall supply certificates of <br /> insurance setting forth the coverage, limits, and endorsements before the City will execute the <br /> contract. The certificate of insurance shall include the City of Decatur, its officers and employees <br /> as an additional insured. In no event shall any failure of the City of Decatur to receive certificates <br /> or to demand receipt be construed as a waiver of the contractor's obligation to obtain and keep in <br /> force the required insurance. <br /> Notice: <br /> It shall be an affirmative obligation upon the contractor to advise the City's <br /> g p Y <br /> Representative at Fax No. (217) 424-2799 or mail to CITY ENGINEER, I GARY K. <br /> ANDERSON PLAZA, DECATUR, IL 62523, within two days of the cancellation or substantive <br /> change of any insurance policy set out above, and failure to do so shall be construed to be a <br /> breach of the contract. <br /> NOTICE: THIS CONTRACT WILL NOT BE AWARDED UNTIL THE <br /> CONTRACTOR HAS PROVIDED THE CITY WITH A CONTRACT PERFORMANCE <br /> BOND AND PROOF OF INSURANCE. <br /> GC-2 CLEAN UP <br /> The Contractor shall at all times keep the premises free from accumulations of surplus <br /> excavated material,waste material or rubbish caused by their employees or work. <br /> Upon completion of the work, the Contractor shall remove all their rubbish from and <br /> about the structures, buildings, and premises, also all their tools, equipment, scaffolding and <br /> surplus materials, and shall leave their work clean, ready for use. In case of dispute, the Owner <br /> may remove the rubbish and surplus materials and charge cost of removals to the Contractor or <br /> separate contractors, if any, in proportion to amounts determined by the Engineer to be just. <br /> See Technical Specification Section 017700 for additional requirements. <br /> GC-3 CONSTRUCTION STAKING <br /> Not Applicable <br /> 23 j <br />
