Exhibit A SMOKE TESTING 1. Locations and color photos of points where smoke is observed escaping from pipe. Evaluation of probable cause. ADDITIONAL AND/OR EMERGENCY INSPECTION/SURVEYS We are prepared to offer our services to the City of Decatur in the inspection and evaluation of their sanitary and storm sewer infrastructure by doing whatever it takes to provide a quality service in a professional, safe and cost-effective manner. We have and will stay focused on the City of Decatur engineering needs; emphasizing long-term, mutually beneficial relationships based on trust and respect. 1. T.V. Inspection/Survey 2. Proper documentation and preparation of all field data. 3. Evaluation of data and recommendations of repair of problem areas. 4. Field engineering surveys for the location of lost or buried manholes including horizontal and vertical locations of relocated sewer lines of which there are no records available. 5. Emergency work is offered on a 24 hour per day, 7 days per week, with no more than a two-hour response time. ASSUMPTIONS/CONDITIONS This Agreement is subject to the following assumptions/conditions: SCHEDULE It is anticipated that the T.V. Inspection survey will take place from June 1,2013 through December 31, 2015. Any additional footage to be televised or smoke testing,manhole evaluations and other services we are asked to perform will be completed in a timely manner as agreed to by the City Engineer. The prices in this contract will be applicable through December 31, 2015. ACCEPTANCE OF CONDITIONS STATEMENT Tele Scan,Inc. hereby affirms that they are in full acceptance of all conditions and requirements and are prepared to execute an agreement with the City of Decatur and perform these services accordingly. 1.
