Exhibit A
Scone of Work
Cin, of Decatur
Assessment of Mound Road Bridges (SN 058-3035, 058-3036,and 058-3037)
URS will perform the following tasks:
I) Review existing plans and inspection reports
2) Conduct site visit to document condition of each bridge for Bridge Condition Report
3) Prepare Bridge Condition Report(BCR) in accordance with IDOT Bridge Condition
Report Procedures & Practices Manual, December 2011
4) Perform project management duties
5) A Project Development Report is not included in the scope of work.
Bridge Condition Report and Inspection
A Bridge Condition Report (BCR)provides a format to develop and document a proposed scope of
%,ork for a structure. The report is submitted to the Bureau of Bridges and Structures for reN ie',N and
approval.
Structures being inspected to produce a BCR outlining the scope of work for a bridge will generally
require more effort than an inspection to meet the periodic mandated safety inspection
requirements. The current condition of a structure can also affect the effort. A structure in poor
condition will usually require more effort than one in good condition. The inspection must be
docwttented in such a manner as to allow a proper scope of work to be determined and approved. A
thorough detailed inspection will reduce the potential need for return trips to the site to secure
additional information during the report preparation and approval process.
Proper documentation of the inspection results is critical to producing a good Bridge Condition
Report. Without proper documentation, it will be difficult to conduct a thorough analysis of the
structure to determine the appropriate scope of work.
URS will document the findings in a SCR for each structure and determine a course of action. The
options available for deck beam bridges are:
1) Bridge Replacement— Replacement of entire bridge.
2) Bridge Reconstruction— At a minimum complete replacement of the superstructure and
may include work on the substructure and foundation.
3) Bridge Rehabilitation —Repair or replacement work on one or more major and/or minor
components of a bridge.
Al
