Exhibit A
4) Bridge to remain in place—The bridge is structurally sound, has adequate load capacity
and meets the minimum width/clearance/geometric criteria to remain in place without
work.
Evaluations required for a deck beam bridge during the analysis process include:
I) Geometric & Hydraulic Capacity **
2) Condition of Deck Beams
3) Condition of Substructure
4) Miscellaneous Checks
5) Stage Construction Feasibility
6) Economics
7) Exterior Constraints
** Hvdraulic Analysis and Scour Evaluation
IDOT requires that a hydraulic analysis be performed for superstructure replacement of for total
hridpc replacement. A hydraulic analysis is not required if only a limited number of beam lines are
replaced.
Statewide Permit No. 12 (SWP 12) authorizes the replacement of bridges over streams under
INDR's jurisdiction draining one square mile or more in urban areas. The new, structure must
provide the same or greater effective waterway opening as the existing structure and an Illinois
licensed professional engineer must detennine and document that the existing structure has not been
the cause of demonstrable flood damage. The project cannot include any appreciable raising of the
roadway approaches.
For the initial structure assessment, it will be asstuned that bridges have not been the cause of
demonstrable flood damage and that existing;hydraulic opening is adequate. Hydraulic evaluation
for scour will be required before final design, but is not included in this scope of work.
Quality Assurance and Quality Control
Work wille in b accordance with the URS Quality Management System. The Bridge Condition
Reports will receive an independent technical review (iTR)performed by a qualified individual who
is not the originator of the work product. The purpose of the ITR is to validate the assumptions,
judgements, opinions, conclusions, and recommendations.
A2
