- 21 - <br /> b) If a full-time employee has exhausted his/her paid <br /> sick leave credits and has been granted a leave of <br /> absence from duty or from the classified service <br /> because of illness or injury, his/her group insurance <br /> coverage shall remain in effect for up to six (6) <br /> months of such leave. If, by reason of policy of the <br /> City of Decatur, the group coverage of such employee <br /> is required to be converted to individual coverage at <br /> a time prior to the time provided herein, the Library <br /> will assume the cost of coverage for the converted <br /> policy of this employee for a period of six (6) <br /> months . <br /> c) The Library will contribute toward the cost of <br /> dependent coverage for all full-time bargaining unit <br /> members $450 . 00 per month for the period May 1, 2012 <br /> through April 30, 2017 . <br /> (d) Retiring employees shall be allowed to purchase <br /> insurance through the medical/hospitalization plan if <br /> the retiring employee pays the cost involved. <br /> Section 2 . Life Insurance <br /> a) The Library shall participate in the City of Decatur' s <br /> group life insurance plan. The Library shall assume <br /> the cost of coverage for each full-time employee. <br /> b) If a full-time employee has exhausted his/her paid sick <br /> leave credits and has been granted a leave of absence <br /> from duty or from the classified service because of <br /> illness or injury, his/her group life insurance policy <br /> shall remain in effect for up to six (6) months of such <br /> leave; and if such illness or injury is certified by the <br /> employee' s physician to be of a terminal nature and if <br /> said employee has retired under the disability <br /> provisions of the Illinois Municipal Retirement Fund, <br /> his/her group life insurance policy shall remain in <br /> effect for up to twelve (12) months. In the event that <br /> the policy adopted by the City of Decatur requires that <br /> the group insurance of such employee be converted to <br /> individual .coverage at a time prior to the time provided <br /> herein, the Library will assume the cost of coverage for <br /> the converted policy of this employee for a period of <br /> six (6) months. <br /> ARTICLE XII <br />
