-� <br /> FI 2012-2013 CllBG SUBRECIPIENT AGREEMENT <br /> THIS AGREEMENT entered this 18th day, of June, 2012 by and between the City of Decatur, <br /> Illinois (herein called the "Grantee") and Workforce Investment Solutions (WIS). (herein <br /> called the "Subrecipient"). Project Partners are identified as: Richland Community College, <br /> Decatur Public Schools Adult Education, Richland Community College-Project Read, The <br /> Community Foundation of Decatur and Macon County, and Economic Development Corporation <br /> of Decatur/Macon County. <br /> WHEREAS, the Grantee has applied for and received funds from the United States Government <br /> the U.S. Department of Housing and Urban Development ("HUD") under Title I of the Housing <br /> and Community Development Act of 1974, Public Law 93-383; and <br /> WHEREAS,the City wishes to engage the Subrecipient and the project partners to assist the City <br /> in utilizing such funds in accordance with HUD guidelines and Federal regulations; <br /> NOW, THEREFORE, it is agreed between the parties hereto that; <br /> I. STATEMENT OF WORK: <br /> A. Activities: <br /> The Subrecipient will be responsible for administering activities for the FY2012-13 <br /> program year in a manner satisfactory to the City and consistent with any standards <br /> required as a condition of providing these funds. Such programs will include the <br /> following activities .eligible under the Community Development Block Grant <br /> Program: <br /> Program Delivery <br /> Activity <br /> Prepare and equip at least 36 low-income individuals for employment. The <br /> partnership will assure that workers who are laid off can return to the workforce with <br /> essential new skills which will allow them to minimize periods of unemployment and <br /> maximize their earning potential. <br /> Prepare participants for entry-level jobs in manufacturing and health care. The <br /> program will target manufacturing positions and Certified Nurse Assistant positions. <br /> Special features include: guidance by employers in need of qualified applicants, <br /> targeting hard-to-serve unemployed persons with multiple barriers, use of <br /> sophisticated assessment to those motivated to succeed, a five-week Basic <br /> Employment Skills course for all participants, sixteen weeks of Job-Specific Training <br /> including classroom sessions, laboratory work, and skill development, and two weeks <br /> of professionally guided Job Search activities. <br /> Subrecipient Agreement (Rev.
