Exhibit A <br /> Detailed Scope of Services <br /> South Shores Drive over Sand Creek Arm of Lake Decatur <br /> Construction Phase <br /> Task 20100—Proiect Management <br /> The following tasks will be performed as part of the project management: <br /> 1. Prepare Electronic Work Authorization Form(eWAF) based on approved manhours and <br /> tasks for the supplement. <br /> 2. Prepare spreadsheets for tracking project. <br /> 3. Update Project Execution Plan (PXP). <br /> 4. Prepare monthly progress report and updates to graph showing actual expenditures <br /> relative to budgeted and submit with the invoices. <br /> 5. Document and maintain a record of all project communication, coordination, transmittals <br /> and submittals. <br /> Tasks 20205 throup-h 20225—Construction Phase— URS <br /> The following tasks will be performed: <br /> 1. Attend Pre-Construction Conference if required. <br /> 2. Respond to requests for information(RFIs) relating to the roadway bridge steel <br /> fabrication and construction. Seventeen(17) hours have been budgeted for this task. (See <br /> Item 6.) <br /> 3. Review shop drawings associated with the roadway bridge. When all drawings are <br /> approved, shop drawings will be distributed to the following: <br /> a. Fabricator <br /> b. IDOT Inspector <br /> c. City Engineer <br /> d. Contractor <br /> e. IDOT Engineer of Materials <br /> f. IDOT District 7 <br /> 4. Respond to construction phase questions relating to the roadway bridge and roadway <br /> plans. Thirty-seven(37) hours have been budgeted for this task. (See Item 6.) <br /> 5. Check construction layout and/or associated items when requested by the City of <br /> Decatur. Thirty-two (32)hours have been budgeted for a 2-person survey crew for this <br /> task. (See Item 6.) <br /> 6. In the event that the total budget allocated for items 2, 4, and 5 is exceeded, additional <br /> hours and fee will need to be negotiated with the City of Decatur. <br /> 4-5-12 A-1 <br />
