Exhibit A <br /> SCOPE OF SERVICES: <br /> Manholes <br /> 1. Physically locate and mark the manholes on the project sewers. (Check the hydrogen sulfide <br /> concentration in the sewer at each manhole.) <br /> 2. Perform a closed level circuit to establish the rim elevations of the manholes on the project <br /> sewers. <br /> 3. Photograph the location of each manhole. Visually inspect the manhole from the surface and note <br /> any irregularities. Measure the depth of each manhole and establish an invert elevation. Check <br /> the hydrogen sulfide concentration in the sewer at each manhole. <br /> 4. From record drawings and measured depths,draft a detail for each manhole. <br /> 5. Review Telescan manhole inspection logs and CCTV files. Mark up manhole drawings with <br /> defects for repair. The City shall furnish current inspection logs and CCTV files for all manholes. <br /> 6. Draft defects onto the manhole detail drawings. <br /> 7. Design the necessary repairs and rehabilitation for each manhole. <br /> 8. Draft the repairs and rehabilitation onto the detail drawings. <br /> 9. Tabulate the manhole repair and rehabilitation quantities. <br /> 10. Write technical specifications for the manhole repair and rehabilitation techniques. <br /> 11. Write special conditions/provisions for specific areas of attention. <br /> 12. BGM will purchase two hydrogen sulfide gas concentration logging samplers for use on this <br /> project. The loggers will be installed in various manholes for sixty days to measure the hydrogen <br /> sulfide concentrations as an indicator of potential corrosion levels. When the monitoring is <br /> complete,the monitors will be turned over to the City. <br /> Trunk Sewer <br /> 1. From record drawings and measured manhole invert elevations, draft a plan and profile for each <br /> sewer at an appropriate horizontal and vertical scale. <br /> 2. Review Telescan CCTV files and reports. Tabulate sewer conditions and defects. The City shall <br /> furnish current inspection logs and CCTV files for all project sewers. <br /> 3. At an estimated 3 locations in the Lake Shore Drive—East,West and 16th St. segments, an <br /> internal inspection will be made to verify the condition of the sewer walls and sewer bottom. The <br /> location of the inspections will be made based on the defects shown on the CCTV files and the <br /> accessibility to the sewer. The inspections will be made upstream and downstream of the entry <br /> manholes. These inspections are classified as confined space entries. The entry will be done with <br /> an experienced and trained 6 man crew. Two men will be inside the sewer, one man at the entry <br /> manhole, one man at the emergency exit manhole, and one man at each of the two ventilation <br /> blowers. He crew will be equipped with a personnel wench, harness,two generators,two <br /> blowers, one air quality meter,two emergency air pacts, and necessary protective ware. <br /> 4. Draft defects,sewer conditions and other information onto the sewer profile drawings. <br /> 5. Design the necessary repairs and rehabilitation for each sewer segment between manholes. <br /> 6. Draft the repairs and rehabilitation onto the sewer profile drawings. <br /> 7. Tabulate the repair and rehabilitation on a schedule of quantities. <br /> 8. Write technical specifications for each of the sewer repair and rehabilitation techniques. <br /> 9. Write special conditions/provisions for specific areas of attention. <br /> Opinion of Probable Cost <br /> 1. An opinion of probable cost will be prepared for each of the project sewers. Most of the items <br /> will be based on estimated quantities and projected unit costs. <br /> Technical Peer Review <br /> 1. Perform an in house peer review by an engineer and technical staff who have not worked on the <br /> project. <br /> A-3 <br />
