r , <br /> REMOVE A SINGLE COMMERCIAL ENTRANCE <br /> The Applicant is hereby granted permission to remove a single commercial entrance by full <br /> depth saw cutting and removing ±31' of existing B-6.24 depressed concrete curb and gutter <br /> and the existing driveway pavement. Then construct new Class "SI" concrete B-6.24 concrete <br /> curb and gutter. Any sidewalk that is removed shall be replaced in kind. Prior to permit <br /> approval all disturbed areas shall be reshaped and seeded on the State highway right-of-way. <br /> This work will be located and constructed in accordance with the Special Provisions, <br /> description and plans attached hereto and made a part of this permit. The material used in this <br /> permit shall be from a State approved source. <br /> This commercial property is designed to serve as access to G.W. Plaza, LLC. If the use of this <br /> entrance is changed, the owner shall revise the driveway as stated in Paragraph Nine on the <br /> back side of Form BT 1045. <br /> DETENTION BASIN OUTLET <br /> The Applicant is hereby granted permission to construct a drainage outlet by installing a 12" o <br /> reinforced concrete culvert pipe (R.C.C.P.) into an existing manhole at Station 1265 + 90. Any <br /> sidewalk that is removed shall be replaced with Class "SI" concrete on the State highway right- <br /> of-way. This work will be located and constructed in accordance with the Special Provisions, <br /> description and plans attached hereto and made a part of this permit. <br /> All labor and material required to complete and maintain this permit are the responsibility of <br /> the Applicant. <br /> When doing the work covered in this permit, the Applicant shall use Traffic Control Standard <br /> 701601, which is attached and made a part of this permit. Standard 701601 is a minimum <br /> requirement and additional or supplemental devices may be required. All Traffic Control <br /> Devices must meet NCHRP-350 requirements. <br /> It is understood and agreed that the Applicant, his successors, or assigns shall not alter or <br /> change the proposed drainage contours or storm water detention basin as shown on the <br /> attached prints. If a change is proposed in the grade, area, or type of surface, the Applicant, <br /> his successors, or assigns shall submit a new site plan to the Illinois Department of <br /> Transportation for review before making any such changes on the property. <br /> If there are any questions concerning the completed construction of the storm water detention, <br /> upon request from the Department, the Applicant shall furnish an as-built plan and <br /> computations to verify compliance with the approved site plan. <br /> The storm water detention design performed under this permit shall be designed by a <br /> Registered Professional Engineer knowledgeable in the storm water detention policies, <br /> procedures and methods required by the Illinois Department of Transportation. <br /> The Engineer preparing the storm water study and storm water detention design shall be <br /> responsible for the accuracy of his work. Issuance of the Highway Permit by the Department <br /> shall not relieve the Engineer of the responsibility for any design errors and/or omissions in the <br /> study and/or detention design. <br /> 2of13 <br />
