r , REMOVE A SINGLE COMMERCIAL ENTRANCE The Applicant is hereby granted permission to remove a single commercial entrance by full depth saw cutting and removing ±31' of existing B-6.24 depressed concrete curb and gutter and the existing driveway pavement. Then construct new Class "SI" concrete B-6.24 concrete curb and gutter. Any sidewalk that is removed shall be replaced in kind. Prior to permit approval all disturbed areas shall be reshaped and seeded on the State highway right-of-way. This work will be located and constructed in accordance with the Special Provisions, description and plans attached hereto and made a part of this permit. The material used in this permit shall be from a State approved source. This commercial property is designed to serve as access to G.W. Plaza, LLC. If the use of this entrance is changed, the owner shall revise the driveway as stated in Paragraph Nine on the back side of Form BT 1045. DETENTION BASIN OUTLET The Applicant is hereby granted permission to construct a drainage outlet by installing a 12" o reinforced concrete culvert pipe (R.C.C.P.) into an existing manhole at Station 1265 + 90. Any sidewalk that is removed shall be replaced with Class "SI" concrete on the State highway right- of-way. This work will be located and constructed in accordance with the Special Provisions, description and plans attached hereto and made a part of this permit. All labor and material required to complete and maintain this permit are the responsibility of the Applicant. When doing the work covered in this permit, the Applicant shall use Traffic Control Standard 701601, which is attached and made a part of this permit. Standard 701601 is a minimum requirement and additional or supplemental devices may be required. All Traffic Control Devices must meet NCHRP-350 requirements. It is understood and agreed that the Applicant, his successors, or assigns shall not alter or change the proposed drainage contours or storm water detention basin as shown on the attached prints. If a change is proposed in the grade, area, or type of surface, the Applicant, his successors, or assigns shall submit a new site plan to the Illinois Department of Transportation for review before making any such changes on the property. If there are any questions concerning the completed construction of the storm water detention, upon request from the Department, the Applicant shall furnish an as-built plan and computations to verify compliance with the approved site plan. The storm water detention design performed under this permit shall be designed by a Registered Professional Engineer knowledgeable in the storm water detention policies, procedures and methods required by the Illinois Department of Transportation. The Engineer preparing the storm water study and storm water detention design shall be responsible for the accuracy of his work. Issuance of the Highway Permit by the Department shall not relieve the Engineer of the responsibility for any design errors and/or omissions in the study and/or detention design. 2of13
