Exhibit A <br /> Detailed Scope of Services <br /> South Shores Drive over Sand Creek Arm of Lake Decatur <br /> Final Design Phase <br /> Task 00100—Project Management <br /> The following tasks will be performed as part of the project management: <br /> 1. Prepare Electronic Work Authorization Form (eWAF)based on approved manhours and <br /> tasks. <br /> 2. Prepare spreadsheets for tracking project. <br /> 3. Prepare Project Execution Plan (PXP). <br /> 4. Prepare for and conduct project kickoff meeting. <br /> 5. Monthly progress report and updates to graph showing actual expenditures relative to <br /> budgeted will be prepared and submitted with the invoices. <br /> 6. A record of all project communication, coordination, transmittals and submittals will be <br /> documented, and maintained. <br /> 7. Schedule staff assignments, monitor budgets and manage development of deliverables <br /> and timeline. <br /> 8. Conduct internal staff and team meetings. <br /> Task 00200—Update Hydraulic Report <br /> The hydraulic report and waterway information table will be updated for the raised bridge. The <br /> original hydraulic report and waterway information table were generated prior to the decision to <br /> raise the bridge for an additional 3' of vertical clearance. <br /> Task 00300— Structure Design <br /> The following tasks will be included in the structural design: <br /> 1. Design/check required beam size for spans established in Phase I. <br /> 2. Design/check concrete slab for beam layout established in Phase I. <br /> 3. Design/check shear stud layout for composite design in both positive and negative <br /> moment regions. <br /> 4. Design/check beam splices <br /> 5. Design/check fixed bearing at pier <br /> 6. Develop/check top of slab elevations <br /> 7. Design/check bridge deck drainage system <br /> 8. Design/check bridge abutments and foundations <br /> 9. Design/check pier and foundation. <br /> 10. Design/check cofferdam seal coat and sheet piling at pier. <br /> 11. Design/check sheet pile wall to be included along north side of roadway. Design will <br /> include sheet pile cap and fencing—This work to be included in Task 20000 <br /> 6-8-11 A-1 <br />
