ATTACHMENT A <br /> FOR <br /> TASK ORDER NO. 18 <br /> FOR <br /> CONTINUING AGREEMENT FOR ENGINEERING SERVICES <br /> Owner: City of Decatur, Illinois <br /> Engineer: Black&Veatch Corporation <br /> Project: South Water Treatment Plant Basin Inspections and Report <br /> Date: April 20, 2011 <br /> DESCRIPTION OF SCOPE OF SERVICES <br /> The scope of services for Task Order No. 18 shall generally consist of the following: <br /> 1. Perform a visual inspection of two pairs of primary secondary basins plus the <br /> concrete walkways between the basins at the South Water Treatment Plant. Each <br /> pair of basins shall be inspected on two separate occasions to allow one set of <br /> basins to be removed from service at a time. The inspections shall be performed <br /> after the basins have been dewatered by Owner's staff. The inspections shall <br /> consist of visual observation of all of the visible features of the basins including <br /> basin equipment,piping, and concrete surfaces. Owner shall supply ladders and <br /> other forms of access as Engineer may require to perform a thorough inspection <br /> of otherwise inaccessible areas. Both inspections shall be performed during two <br /> separate 8-hour periods by a three-person team consisting of two of Engineer's <br /> staff plus a representative from the existing basin equipment manufacturer(Infilco <br /> Degremont, Inc). <br /> 2. Prepare a written letter report to communicate to Owner a summary of the <br /> Engineer's observations during the inspection, the Engineer's recommendations <br /> for repairs to the basins and basin equipment, and the Engineer's opinion of <br /> probable costs for the recommended repairs. Engineer shall initially submit three <br /> copies of a draft letter report for Owner's review. Following receipt of Owner's <br /> comments,Engineer shall prepare and submit three copies of Engineer's final <br /> letter report. <br /> 3. Engage the services of a representative from the existing basin equipment <br /> manufacturer(Infilco Degremont, Inc.)to assist with the inspection and report for <br /> the existing basin equipment. <br /> END OF ATTACHMENT A <br /> 2 <br /> City of Decatur Task Order No.18 <br /> Rev.04-07-11 <br />
