� , ' ' • �� ��� ���' <br /> Pricing <br /> Testing and Cleaning as described above............................................................................... $45,772.00 <br /> Comments and Exceptions <br /> • Additional labor required to implement any repairs or work outside of the above work scope <br /> will be accomplished at your written direction and authorization, and will be billed as <br /> additional work. <br /> • Delays in performing the work out of the control of Siemens will be considered and billed as <br /> extra work to owner's account. <br /> • Delays in the performance of work beyond the control of Siemens or its subcontractors will be <br /> billed as additional work. Siemens will not be responsible for the completion of any additional <br /> "required" customer documentation regarding delay time in order to collect delay time <br /> charges as a condition of the acceptance of this quotation by the customer. <br /> . Siemens and its subcontractors reserve the right to work only when safety practices are <br /> acceptable to Siemens. Costs associated with any delays caused by disposition of <br /> substandard safety conditions will be outside the scope of this quotation and invoiced as <br /> additional work. <br /> • Work performed around energized equipment with a rating of 1000 volt and above will require <br /> two (2) OSHA CFR1910.269 trained personnel minimum. <br /> • Applicable state and local sales taxes will be charged in addition. <br /> Offer Validity <br /> This offer is valid for 30 days unless otherwise extended, modified or withdrawn, in writing, by <br /> Siemens. The return of a purchase order to Siemens during such validity period, along with credit <br /> approval, will be sufficient to form an agreement on the terms and conditions of this offer. <br /> Safety Training of Siemens Field Personnel <br /> All Siemens field personnel completed internal annual safety training. Furthermore, alt Siemens <br /> field personnel received training to comply with the new OSHA CFR1910.269 Electrical Safetv <br /> Standard that sets minimum safety rules and practices forthe design, operation, and <br /> maintenance of high-voltage systems (over 600 volts). A morninq safetv meetinp for Siemens <br /> emplovees and subcontractors is to be conducted bv Siemens each dav while on site. <br /> All Siemens personnel and their subcontractors shall adhere to the Siemens Division Safety <br /> Guidelines, FS101. Copies can be made available upon request for client review.
