2. Easter Seals shall use information gathered from various sources to determine the <br /> applicant's eligibility for ADA paratransit services: <br /> • the information supplied on the application; <br /> • the additional information gathered during the evaluation interview; <br /> • any medical information supplied by the applicant's doctor, included with the , <br /> application; <br /> • when necessary, the answers to follow-up questions addressed to the applicant's <br /> doctor; <br /> • if the functional mobility assessment is used, the detailed observations of the <br /> evaluator of the applicant's physical and cognitive functioning; and <br /> • any other information gathered before, during, or after the interview and functional <br /> mobility assessment that pertains to the applicant's eligibility. <br /> Then, using reasoned judgment and adhering strictly to the specific eligibility standards <br /> developed for the Decatur area, Easter Seals will determine if the applicant is eligible for <br /> ADA paratransit services and, if eligible, ascertain the appropriate category of eligibility: <br /> unconditional, conditional, or temporary. <br /> 3. Easter Seals shall process each eligibility determination and submit that determination as <br /> a recommendation to the City. <br /> a. Easter Seals shall prepare accurate and thorough documentation of its reasons for <br /> making each eligibility determination, including the reasons for determining the <br /> category of eligibility. Effective documentation may require the use of audio and <br /> video recordings in some cases. <br /> b. Easter Seals shall submit its eligibility determination for each applicant to the City as <br /> a recommendation. The City shall be responsible for making the final decision on <br /> each application, and communicating that decision to the applicant. <br /> Easter Seals shall submit its recommendation to the City no later than 15 calendar days <br /> after it completes an eligibility evaluation. <br /> 4. Easter Seals shall prepare monthly statistical reports and billing statements, and an annual <br /> suminary report of all activities related to this contract. <br /> a. Monthly reports shall be submitted no later than the 15th day of the following month. <br /> The monthly report shall indicate the following: <br /> • The number of applications received. <br /> • The number of eligibility evaluations scheduled. <br /> • The number of eligibility evaluations completed, with applicants' names and <br /> dates of evaluations. <br /> • 1' in of the determinations made/com leted durin the month se arated into <br /> A ist g p g , p <br /> eligibility categories. <br /> 7. <br />
