• A detailed invoice for all billable activities for the month. <br /> • Other reporting requirements as notified by the City. <br /> b. Annual summary reports shall be submitted no later than January 31 st of the <br /> following year. The annual summary report shall indicate the following: <br /> • The number of applications received. <br /> • The number of eligibility evaluations scheduled. <br /> • The number of eligibility evaluations completed, with applicants' names and <br /> dates of evaluations. <br /> • A listing of the determinations made/completed during the year, separated into <br /> eligibility categories. <br /> • Other reporting requirements as notified by the City. <br /> PART II. ADA TRAINING <br /> All services provided by Easter Seals under this Agreement shall be performed by fully qualified <br /> and trained personnel employed by Easter Seals. All personnel assigned to the project shall have <br /> received Americans with Disabilities Act (ADA) training as specified by the City. <br /> The City shall reimburse Easter Seals for the direct expenses for ADA training for an agreed <br /> upon number of Easter Seals' employees assigned to this project, when such training is specified <br /> and requested by the City. Eligible training expenses would include registration fees, travel, <br /> lodging and meals. <br /> 8 <br />
