The final deliverabte will consist of an ESRI Geodatabase containing NASSCO <br /> MACP and PACP-compliant attributes. A compilation of standard NASSCO forms for <br /> individual sewer manholes_ Collectively, the video and MACP/PACP reports will be <br /> used to perform a conditional assessment of each sewer manho�e and these data will <br /> be reported to the City. <br /> a. All dimensions of the manhole <br /> b. All sewer line tie-ins and sizes <br /> c. Direction of flow/depth <br /> d. Depth and type of debris <br /> e. Location and type of defects <br /> f. Environmental conditions, water and atmosphere temperatures <br /> g. Evaluafion of existing conditions found <br /> h. Video of the MH in MPEG format <br /> D. ASSUMPTIONS/CONDITION3 <br /> This Agreement is subject to the following assumptions/conditions: <br /> SCHEDULE <br /> It is anticipat�l that the Manhole Inspection survey will take place�rom July 1, 2009 <br /> through April 30,2010. The prices in this contract will be applicable through April 30, <br /> 2012 <br /> E. ACCEPTANCE OF CONDITIONS STATEMENT <br /> Te1e Scan, Inc. hereby affirm that they are in full acceptance of all conditions and <br /> requirements and are prepared to execute an agreement with the City of Decatur and <br /> pertorm these services accordingly. <br /> 1. The work shall be performed in compliance with applicable OSHA requirements. <br /> 2. Alt insurance requirements shall be met. <br /> F.
