Amendment to <br /> Agreement C08L0198 <br /> Whereas, the City of Decatur, subsequently referred to as "Client,"and Hanson Professional Services Inc., <br /> subsequently referred to as "Hanson,"have previously entered into a Professional Services Agreement dated <br /> December 29, 2008 in connection with the Lake Decatur Dam Repairs subsequently referred to as "Project,"and <br /> Whereas, the Client has ordered certain changes to the services being provided by Hanson for the Project. <br /> Now,therefore,this Amendment to the Agreement(Amendment No. 1) is made this 3rd day of April, 2009 to <br /> revise the Scope of Services and Cost of Services as provided herein. All other terms and conditions of the <br /> Agreement remain unchanged. <br /> The Scope of Services is modified as follows: <br /> 1. Item 2.b of the original Scope of Services, North and South Sections of the Spillway, is modified to <br /> provide additional plans and specifications in order to complete the work for Gate Seal Heater Removal <br /> and Gate Seal Replacement; and a credit for deleting plans and specifications related to the Hydraulic <br /> Cylinder Repairs. <br /> 2. Item 2.d of the original Scope of Services, Gated Outlet Structure, is modified to provide additional <br /> services for work to rehabilitate the Sluice Gate Machinery. <br /> The Cost of Services is modified as follows: <br /> Cost of Services in Agreement $83,400 <br /> Total of Previous Amendments $0 <br /> Increase this Amendment <br /> • Gate Seal Heater Removal $1,870 <br /> • Gate Seal Replacement $2,400 <br /> • Sluice Gate Machinery Rehabilitation 23 430 <br /> Cost of Services with all Amendments $110,100 <br /> Client and Hanson hereby agree to and accept the terms as stated herein. <br /> Hanson Professional Services Inc.
