, Attachment A
• Work Order No. l (4/20/09)
1.9 Perform hydraulic and scour analysis. Checks will be made to determine if
IDNR-OWR Statewide Permit 12 applies. A Hydraulic Report will be
prepared. Since the bridge is located in an urban area and has a drainage area
of 15.38 square miles, the report will need to be submitted and approved by
IDNR Office of Water Resources.
1.10 Conduct courthouse search and tax map review to establish preliminary
property lines and ownership and to estimate required right-of-way.
1.11 Conduct a single public meeting presenting the results of the bridge type
study and to discuss other issues such as pedestrian/bicycle access and road
closure, if required.
1.12 Coordinate and obtain soil borings for the structure. SKS Engineers will act
as a subcontractor to URS to perform this work.
1.13 Prepare TS&L for submittal to IDOT District 7 and the City of Decatur.
1.14 Submit information to the U.S. Army Corps of Engineers and obtain
authorization for construction. The manhour estimate is based on obtaining a
Nationwide Permit. If the Corps determines that an individual permit is
required, additional manhours may be required for coordination and
submittals.
1.15 Collect the required information and submit the Environmental Survey
Request(ESR). Since the job will use federal funds, it is anticipated that
IDOT will issue the cultural and biological sign-offs for the project.
1.16 Collect data required for inclusion in the Project Development Report. This
data includes traffic information, accident history, required right-of-way,
environmental information, maintenance of traffic, public involvement and
other coordination. The Project Report will be prepared and submitted to
IDOT District 7.
1.17 Conduct Independent Technical Review of project deliverables including the
Bridge Type Study, Project Development Report and TS&L.
1.18 Perform project management tasks which include, setting up the project in
the URS accounting system, preparing a safe work plan for field survey,
preparing the project execution plan, conducting the project kickoff ineeting,
general project administration and invoicing, and coordination with IDOT the
City of Decatur, and the Decatur Sanitary District.
2
