Levels of Accomplishment- Performance Goals <br /> In addition to the normal administrative services required as part of this Agreement, <br /> the Subrecipient agrees to provide the following levels of program services with a <br />; completion date as follows: <br /> Total Units Com letion Date <br /> a. 750 persons will be <br /> assessed April 15, 2009 <br /> b. 300 new students will <br /> be enrolled in the proj ect <br /> c. 340 new volunteers will <br /> be recruited and trained <br /> d.14,000 hours of one-on— <br /> one tutoring will be <br /> documented <br /> C. Staffing <br /> Administrative Assistant <br /> 2-Literacy Resource Managers <br /> Volunteer Coordinator <br /> D. Performance Monitoring <br /> The City will monitor the performance of the Subrecipient ANNUALLY against <br /> goals and performance standards required herein in accordance to the CDBG <br /> recipient monitoring plan. Substandard performance as determined by the City will <br /> constitute noncompliance with this agreement. If action to correct such substandard <br /> performance is not taken by the Subrecipient within a reasonable period of time as <br /> specified by the City and in accordance to the CDBG monitoring Plan, then, after <br /> being notified by the City, contract suspension or termination procedures will be <br /> initiated. <br /> II. TIME OF PERFORMANCE <br /> Services of the Subrecipient shall begin when given a Notice to Proceed by the Economic and <br /> Urban Development Department-Neighborhood Outreach Division of the City. The term of this <br /> Agreement and the provisions l�erein shall end on April 15, 2009. Program income proceeds (if <br /> applicable) shall be accounted for quarterly. All program income will revert to the City of <br /> Decatur, to be utilized for future funding activities. <br /> Subrecipierzt Agreenie�zt(Rev.12/07.•RDI) Page 2 of 18 <br />
