The City of Decatur operating the water works system with the tank off-line without any as- <br /> sistance from Tank Industry Consultants. <br /> 2. The City of Decatur providing a ladder(or other means)to access the exterior tank ladder, if <br /> the bottom of the tank ladder is more than 12 feet above the ground. <br /> 3. Performing the work when the temperature is above 35°F,the wind velocity is 15 miles per <br /> hour or less, and there is no precipitation. <br /> 4. Performing the field work prior to October 30, 2007. The field work will be scheduled when <br />' written authorization to perform this work has been received. <br /> 5. The City of Decatur furnishing a ground man during the work. <br /> 6. The City of Decatur supplying TIC with all required sleeves and attachments for any safe- <br />� climbing devices on the tank ladders in order for these devices to be tested for proper opera- <br /> tion. <br /> 7. The City of Decatur may not accompany TIC's personnel during the evaluation of the tank <br /> surfaces above ground level. TIC's rigging and safety equipment is intended for and is to be <br /> used by only TIC personnel. <br /> 8. The City of Decatur taking samples and performing all required testing prior to placing the <br /> tank in service. <br /> 15. Using the enclosed agreement, the City of Decatur's request for proposal, and this proposal <br /> as the contract form to perform this work. Specifically included as part of this fee is the <br /> limitation of liability listed in this agreement. <br /> 16. The City of Decatur will be invoiced when the engineering report is issued. Payment shall <br /> then be made within 30 days. <br /> If any of these terms are not met by The City of Decatur, chazges for any additional time,mile- <br /> age, per diem, interest (1-1/2%monthly), taxes, insurance or mobilizations incurred by TIC due <br /> to such failure may be added to the fee. <br /> SSPC QP 5 CERTIFICATION <br /> Tank Industry Consultants is recognized by SSPC: The Society for Protective Coatings as a Cer- <br /> tified Inspection Company.
