b. The annual summary report shall indicate the following: <br /> • The number of applications received. <br /> • The number of eligibility evaluations scheduled. <br /> • The number of eligibility evaluations completed, with applicants' names and <br /> dates of evaluations. <br /> • A listing of the determinations made/completed during the year, separated into <br /> eligibility categories. <br /> • Other reporting requirements as notified by the City. <br /> Annual summary reports shall be submitted no later than January 31 st of the following <br /> year. <br /> PART III. ADA TRAINING <br /> All services provided by Macon Resources under this Agreement shall be performed by fully I <br /> qualified and trained personnel employed by Macon Resources. All personnel assigned to the ', <br /> project shall have received training as specified by the City under this Agreement. The City ' <br /> shall reimburse Macon Resources for the cost of such training for an agreed upon number of <br /> Macon Resources' employees. Eligible costs shall include registration fees, travel, lodging and <br /> meals. <br /> 10 <br />
