4. WORK SCHEDULE AND TERM OF AGREEMENT <br /> The services of Macon Resources are to begin on January 1, 2007. Starting on that date, Macon ' <br /> Resources shall assist the City in developing specific ADA eligibility standards for persons with <br /> disabilities who apply for door-to-door paratransit services. When the City determines that this <br /> part of the scope of services (developing specific ADA eligibility standards) has been completed, <br /> Macon Resources shall begin providing rider eligibility determination services on an ongoing <br /> basis. <br /> The term of this A eement shall be from Janu 1 2007 throu December 31, 2009. <br /> �' �'Y > > � <br /> 5. DELIVERABLES <br /> « �, <br /> Deliverables to be submitted to the City shall be as described in Appendix A, Scope of Work , <br /> and all such deliverables shall be submitted as per the schedule specified therein. <br /> 6. COMPENSATION AND METHOD OF PAYMENT <br /> The City shall reimburse Macon Resources for satisfactory performance of the services described <br /> in Part I - Developing Eligibility Standards of Appendix A, as follows: $35.00 per hour per <br /> person for all services required under Part I. The total amount reimbursed for Part I shall not <br /> exceed $3,500. Macon Resources shall be obligated to complete the services described herein. <br /> However, Macon Resources shall not be required to perform services beyond those described <br /> without an increase in the stated contract cost. In the event of substantial changes in the tasks, <br /> objectives or character of work described herein, the total compensation shall be adjusted only <br /> by mutual agreement of the City and Macon Resources. <br /> The City shall reimburse Macon Resources for each eligibility determination performed, as <br /> described in Part II — Paratransit Eligibility Determinations of Appendix A, as follows: $50.00 <br /> for each eligibility determination performed and properly documented. There shall be no <br /> maximum amount budgeted for this part of the scope of work. The amount reimbursed to Macon <br /> Resources shall be entirely dependent on the number of applicants referred to Macon Resources <br /> and the number of eligibility determinations performed and properly documented. <br /> The City shall reimburse Macon Resources for eligible direct expenses under Part III — ADA <br /> Training of Appendix A, as follows: actual expenses of ADA training for Macon Resources <br /> employees, including registration fees, travel, lodging and meals. <br /> Compensation shall be paid after the City's receipt of Macon Resources' invoices, prepared in <br /> such form and supported by such documents as the City may reasonably require. Macon <br /> Resources shall submit monthly invoices for work actually performed. All Macon Resources <br /> invoices shall be supported by a statement of costs incurred in accordance with this Agreement, <br /> itemized by hours worked by individual and other direct expenses under Part I, and itemized by <br /> the names of applicants for whom eligibility determinations were performed under Part II, and <br /> itemized by direct expenses for ADA training under Part III. The City shall reimburse Macon <br /> Resources by check within 30 calendar days of receipt of Macon Resources' invoices. <br /> 2 <br />
