FIVE YEAR SUPPORTIVE HOUSING PLAN
FiNAL REPORT
Five Year Supportive Housing Plan for Chronically Homeless Persons
Decatur, Illinois
Final Report
I. Introduction and Backqround
The Neighborhood Housing Development Corporation of Decatur, Illinois (NHDC)
presents this five year plan to provide permanent supportive housing for persons in
Decatur and Macon County who are chronically homeless. This plan was developed in
collaboration with the Macon County Homeless Council and the Department of
Economic and Urban Development of the City of Decatur and funded under a grant from
the Corporation for Supportive Housing. NHDC also acknowledges Dove, Inc. and
Heritage Behavioral Health Services, who served as key partners in the completion of
this plan.
Purposes
This project has three primary purposes:
1. To identify and assess the level of chronic homelessness in Decatur and Macon
County, Illinois.
2. To create a strategy to end chronic homelessness and develop an action plan to
create permanent support housing for persons who are chronically homeless
over the next three to five years.
3. To leverage a $500,000 HOME grant from HUD to the City of Decatur to
maximize housing production and attract additional public and private resources.
' Context The project is consistent with the national goal of the federal government to
end chronic homelessness within ten years. In pursuit of that goal, a large number of
local and state governments have developed or are developing "Ten-Year Plans to End
Chronic Homelessness" in cooperation with the federal Interagency Council on
PAGE 1
