FIVE YEAR SUPPORTIVE HOUSING PLAN
FINAL REPORT
have medication weekly to increase medication compliance. She has applied for ,
SSI, but has been denied and is now appealing. She applied for Earnfare and is
awaiting a decision. She has been diagnosed with diabetes that was previously
untreated. She is now receiving healthcare and medications. She continues to
prostitute for income and has an upcoming court case. She may go to the
Department of Corrections this time as she is on probation. She continues to use
crack, but use has decreased. She finds it difficult to be honest about her use and
prostitution. She is enrolled in SA treatment but is not consistent with follow-up. The
client does follow-up with the MISA Team, but there are times that she cannot be
found.
Descriation of Proaosed Permanent Housin Models
Based upon the needs and characteristics of the target population, detailed descriptions
of the three proposed permanent housing models fiave been developed. These models
(contained within the appendix) are designed to establish basic requirements and
provide guidance to supportive housing developers in the planning and structuring of
future developments. Each model, which can be replicated in response to increased
demand, is summarized below.
• Safe Havens will provide permanent supportive housing for chronically homeless
individuals in a low demand residential facility. These units will be targeted to
Typical Resident#1, the hardest-to-serve individuals who are most resistant to '
intervention and treatment. Large single family homes or small buildings which
offer shared living areas and private or semi-private sleeping spaces are
desirable. Given the needs of the population to be served, each facility will have
on-site staff supervision and provide outreach to support services. Residents will
not have leases and will be atlowed to remain in the facility indefinitely as long as
they abide by the rules. These facilities will be operated on an ongoing basis by
local agencies, which provide services to the chronically homeless.
PAGE 29
