FIVE YEAR SUPPORTIVE HOUSING PLAN <br /> FINAL REPORT <br /> V. Proposed Permanent Supportive Housinq Develouments <br /> The first phase of the supportive housing plan for the chronically homeless includes <br /> three proposed developments to be completed under the $500,000 HOME Grant <br /> awarded to the City in 2004. Of this amount, 5% of the grant will be used for CHDO <br /> operating fees. These projects will provide up to 30 permanent supportive housing units. <br /> Detailed project descriptions and financial proformas for each of the proposed <br /> development will be provided to the City prior to funding. <br /> PAGE 3$ <br />
