. 1 , Attachment A to Work Order No.1 Condit Street Subway;Sidewalk Rehabilitation 1) URS will provide the following services: a) Survey new roadway construction to west for project tie in and any other changes since URS's last survey was completed in 1999. Manhole&inlets under the bridge will be opened and cleaned by the Client so URS can survey to determine pipe network and inverts. b) Design&detail two(2)new inlets under bridge on south side of road. Inlets will include one(1)Type A and one(1)Type B both with a Type 3 frame and grate. The Type B inlet will be connected to the existing pipe network at the location of the inlet removal. c) Design and detail 5'minimum wide,5"PCC raised sidewalk on south side to meet 5%minimum slope ADA requirements. Provide 10'minimum vertical clearance for sidewalk under bridge. d) Design and detail cast-in-place L-Type retaining wall with stepped footing to retain fill for sidewalk on south side. Weep holes will be provided for draining the fill supporting the sidewalk. e) Detail block-outs at top of retaining wall to provide sidewalk drainage. Sidewalk will have 2%maximum cross slope. � Provide railing/fencing on top of retaining wall. Railing/fencing type to be determined as Client's project budget allows. g) Provide details for two(2)- 12'lanes minimum to provide room for retaining wall,sidewalk and two(2)inlet structures. Road to be closed during construction to vehicular and pedestrian traffic. h) Provide details for new curb and gutter(B-6.18)on south side to match existing construction at ends of project. i) Include provisions for patching existing light receptacles in south abutment breast wall. j) Provide Alternate Bid details and quantities to replace sidewalk on north side within the project limits if determined that cost can be added to ClienYs project budget. k) Provide Altemate Bid details and quantities to patch spalling concrete on northwest retaining wall face if determined that cost can be added to ClienYs project budget. I) Proved one(1)set of 11"x17"plans sealed by a Registered Professional Engineer in the State of Illinois. m) Provide an AutoCAD(2004 version)copy of the plans converted from Microstation DGN Drawings. n) Provide a Microsoft Word copy of the Standard and Supplemental specifications for the Project Manual. The Standard and Supplemental specifications will reference the Illinois Department of Transportation �, Standard Spec�cations for Road and Bridge Construction and/or the City of Decatur Construction Standards. Other than the Standard and Supplemental specifications,the Project Manual will be j produced by the City. I o) Provide quantities using IDOT pay items. '� p) Provide Engineer's construction cost estimate. �i 2) The Client will: � a) Coordinate the repair of the existing fencing on retaining walls at each of the four(4)quadrants from the bridge. I b) Coordinate cleaning of the ewsting retaining wall and abutment breast wall faces. c) Obtain any required permits&meet with other organizations impacted by the project. d) Be responsible for all lighting,electrical&signing issues for this project. e) Clean and open the existing inlets and manholes in a timely manner for the survey crew. fl Determine from the their inspection, if any other manholes, inlets and/or pipes within the project limits are to be replaced.
