� :. �. <br /> 1 <br /> HOME PROGRAM <br /> SUPPORTIVE HOUSING AGREEMENT <br /> THIS AGREEMENT, entered into on the__1�day of ���ZC U'-' , 2005 by and between <br /> the City of Decatur, Illinois, hereinafter called the "City", and Decatur Cares Rescue Ministries., <br /> hereinafter called"DCRM", organized under the laws of the State of Illinois. <br /> WHEREAS, the City has entered into an agreement with the U.S. Department of Housing and <br /> Urban Development, hereinafter called "HUD", for the purpose of conducting a HOME Program, <br /> and; whereas the City may commit HOME funds for supportive housing projects; and whereas <br /> DCRM submitted a proposal dated September 21, 2004 to conduct activities eligible under the <br /> HOME program guidelines and requested a grant of HOME funds not to exceed 28 000 in <br /> HOME funds to conduct activities under the stated proposal. <br /> NOW, THEREFORE, the parties in consideration thereto mutually agree and covenant as <br /> follows: <br /> I. SCOPE OF SERVICE <br /> A. DCRM shall use the funds as outlined in the project description attached hereto as <br /> Appendix A, for eligible costs of construction, materials, fees and other eligible items <br /> associated with the development of HIS House, rehabilitation of a 4-bedroom single- <br /> family house to be used as a supportive living environment for up to eight formerly <br /> homeless men who have completed a transitional substance abuse or re-entry program. <br /> B. DCRM may receive up to $28,000 from the City under the HOME Program to <br /> implement the proposed activities. Funding will be dispersed based upon actual project <br /> expenditures. <br /> C. DCRM shall not obligate or expend any funds under the terms of this agreement <br /> until a written Notice to Proceed has been issued by the City. <br /> II. DURATION <br /> The term of this agreement and the provisions herein shall end one year from the date of <br /> this agreement unless an extension is granted by the City. A request for any extension to <br /> this term of agreement shall be made in writing and shall include an explanation of why it <br /> is required. The City shall inform DCRM within three weeks of the receipt of the request <br /> for extension as to whether it has been granted. <br /> 1 <br />
