i - � <br /> EXECUTIVE SUMMARY <br /> The City of Decatur's 2004/OS Annual Action Plan documents the activities proposed to be <br /> undertaken during year five of its five-year Consolidated Plan. The City, a federally designated <br /> entitlement community,receives funding under the Community Development Block Grant <br /> (CDBG)program and the Home Inveshnent Partnerships (HOME)program based on a formula <br /> allocation. For the 2004 program year, the City expects to receive $1,772,000 in CDBG funding <br /> and $601,669 in HOME funding to support activities which benefit very low-, low-, and <br /> moderate-income persons. These grants will be used in conjunction with program income, <br /> carry-over funds, and other funding sources to complete programs and projects to further the <br /> goals and obj ectives of the Consolidated Plan. <br /> Part One of this document includes the Standard 424 form and summary documents including the <br /> funding resources, home match requirements, and fiscal year 2004 CDBG/HOME allocations. <br /> Part Two outlines the City's strategy for use of these funds, in conjunction with those of other <br /> agencies, to meet the needs outlined in the Consolidated Plan for housing,public services, <br /> economic development, and infrastructure. It also states the manner in which the City intends to <br /> address other areas covered in the Consolidated Plan including barriers to affordable housing, <br /> antipoverty strategy, coordination of resources, and program monitoring. <br /> Part Three, the Appendices, includes summary tables for the housing and community <br /> development performance goals, the public housing needs, and the listing of proposed projects. <br /> Other appendices provide a listing of other community resources, the required certification <br /> forms, maps which locate the various program areas, and the City's organizational structure. <br /> In summary, the City envisions the accomplishment of its goals through programs that provide <br /> for the housing needs of low- and moderate-income persons; provide supportive housing for the <br /> homeless and those with special needs; assist the disabled with improved access and mobility; <br /> provide emergency repairs to enable families to remain in their homes; create jobs which benefit <br /> very low-, low-, and moderate-income persons; and address a variety of community needs with <br /> infrastructure and rehabilitation programs. The City seeks to continue to implement programs <br /> which support our community's lifestyle and improve the quality of life for all citizens of <br /> Decatur,Illinois. <br /> 1 II <br /> � <br />
