r , , , <br /> ���������T�� <br /> s �, � <br /> ADMINISTRATIVE SERVICES AGREEMENT <br /> This agreement specifies the services to be provided by Consociate Group <br /> "ADMINISTRATIVE COMPANY" to The City of Decatur, Illinois "Employer" <br /> regarding the administration of the Employer's Flexible Benefit Plan for the plan <br /> year beginning January 1, 2004 through December 31, 2004 with subsequent <br /> plan years beginning January 1St <br /> Administrator <br /> The Employer shall be the Plan Administrator and ADMINISTRATIVE <br /> COMPANY shall be engaged as a subcontractor in the performance of <br /> administrative services for the plan. <br /> Services to be provided by ADMINISTRATIVE COMPANY <br /> 1. Plan Update Service. This service includes a plan review and amendment <br /> documents if needed. <br /> 2. Employee Enrollment Assistance. This service includes educational <br /> materials such as a CD, brochures, and enrollment forms. Enrollment <br /> packets including claim forms and instructions for filing are provided to all <br /> participants. <br /> 3. Employee Eliqibilitv Chanqes. We will make changes in employee <br /> eligibility as provided to us by Employer. Our staff will process all changes <br /> properly reported to us prior to five working days of your payroll date. <br /> 4. Employee Account Management. Employee contributions for each pay <br /> period are added to employee's account balances. Services also include <br /> account balance tracking, claims adjudication, and monthly year to date <br /> reports. Employee account management services are available in one of <br /> three methods, Rapid Claims, Client Bank Account, or Check Register. <br /> Each is described in detail on the attached FSA Account Options chart. <br /> Review these options and elect your service method at the bottom of this <br /> agreement. <br /> 5. Participant Assistance. Employees have 24-hour access to their accounts <br /> via the internet at www.consociategroup.com and daytime access to our <br /> FSA call center at 217-423-7788.
