Secure legal review of the Flexible Benefit Plan <br /> Document and amendments and Summary Plan Description from <br /> Employer's legal counsel or contract vendor such as CORBEL. <br /> 2. Enrollment. Complete new plan year enrollment procedures completely <br /> and on a timely basis as described in the attached Enrollment Kit. <br /> 3. Eligibilitv Chanqes. Notify ADMINISTRATIVE COMPANY of changes in <br /> employee eligibility at least 5 business days prior to the first payroll date <br /> affected. ADMINISTRATIVE COMPANY must be notified by email at <br /> ruthtCa�consociateqroup.com or fax (217) 233 - 0103 on the prescribed <br /> form. A printed copy of the prescribed form is included in this packet. <br /> 4. Discriminatorv Plans. Initiate any action required in the event the plan(s) <br /> become discriminatory. <br /> Fees for Services <br /> 1. Participant Service Fee. This fee includes changes to employee eligibility, <br /> employee account management and participant assistance. The fee is <br /> $2.50 per month per participant or $100 minimum. <br /> 2. Annual Compliance Fee.
