,, , _� CONTRACT EXTENSION AGREEMENT ADDENDUM BETWEEN CITY OF DECATUR AND GUIDANCE PATHWAY SYSTEMS INC. FOR PROFESSIONAL SERVICES PROJECT: Planning, Design, General Specifications and Technical Services Concerning a Sign System for the City of Decatur: EXTENSION AGREEMENT THE Original AGREEMENT was made and entered into on July 21St°f 2003 by and between Guidance Pathway Systems, Inc., hereinafter referred to as "CONSULTANT" and the City of Decatur, Illinois, a municipal corporation, hereinafter referred to as "CITY". This extension agreement addendum follows the terms; conditions and covenants of above said document. PURPOSE OF AGREEMENT CITY intends to continue and extend its contract with CONSULTANT to provide Professional and Technical Services and the payment for those services by CITY as originally set forth. The original agreement with the exception of a time extension for completion of the project tasks as identified shall consist solely and entirely of the original July 21�, 2003 Contract. The original contract includes all agreed upon terms and conditions with the exception of a time extension. The time extensior for this contract shall be extended to May 1 St 2004. All of the services to be performed as provicied in original contract'Exhibit A"shall remain (See attachment A). The ability to modify the time limit for completion of the contract is identified in original contract provisions 3.1 "Time of Performance"and contract provision 3.10"Modification" (See attachment E3).
