�- <br /> �� . <br /> aggregate, for bodily injury, death and property damage to any one person and subject <br /> to the same limitations for any one occurrence. The comprehensive general liability <br /> policy shall include coverages for premises and operations, a blanket contractual liability <br /> coverage which includes the indemnification agreement contained in the next preceding <br /> paragraph, broad form property damage, independent contractors and personal injury <br /> liability coverage. The policy issued for the aforedescribed coverages shall name the <br /> City of Decatur, Illinois, as an additional insured. Such insurance shall not be canceled <br /> prior to termination of this Lease. <br /> 9. Certificate of Insurance. Prior to the commencement of the terms of the <br /> Lease, the Center shall file with the City a Certificate of Insurance, in a form acceptable <br /> to the City, signed by an authorized agent of the insurance company or companies <br /> writing the msurance described in said Certificate showmg complete coverage of all <br /> insurance required in this Lease. Such Certificate shall certify to the following: Name <br /> and address of party insured; name and address of authorized agent executing such <br /> Certificate; the description of the type of insurance, and the coverages afforded <br /> thereunder, the insurance policy numbers; the limits of liability of such policies and the <br /> date of expiration of such policies. Further, the insurance companies shall certify that <br /> such policies shall not be modified, canceled or terminated until after no less than thirty <br /> (30) days written notice to the City. The Certificate shall specifically state that (1) the <br /> insurance certified therein contains contractual liability coverage for the indemnity <br /> agreement contained in this agreement, subject to the terms, conditions and exclusions <br /> of the policy, and (2) the City of Decatur, Illinois is named as an additional insured on <br /> the comprehensive general liability and property damage insurance described above. <br />
