r � <br /> 4.0 PAYMENT FOR SERVICES <br /> 4.1 COMPENSATION <br /> The CITY shall pay the CONTRACTOR for work performed and for materials furnished <br /> on the basis set forth in"Attachment B". The amount of compensation to be paid is a <br /> "time and materials" sum which includes all items of labor, time, materials, supplies, <br /> equipment rental, and other costs, as necessary, furnished or used in the completion of <br /> this project, at the unit prices listed in"Attachment B". The total amount of compen- <br /> sation to be paid by the CITY to the CONTRACTOR shall not exceed $16,876.95 <br /> without the prior written permission of the CITY. <br /> 4.2 INVOICES <br /> The CONTRACTOR shall prepare a detailed invoice for the work performed and the <br /> materials supplied and submit it to the CITY at the completion of the project. All costs <br /> charged by the CONTRACTOR shall be supported by properly executed payrolls, time <br /> records, invoices, contracts or vouchers documenting in proper detail the nature and <br /> propriety of the charges. The CITY shall make payment to the CONTRACTOR within <br /> thirty(30) days after receipt of the completed invoice. <br /> 4.3 CHANGE ORDERS <br /> The CONTRACTOR shall not perform any work in addition to that outlined in this <br /> Agreement without prior written authorization by the CITY. If the CITY has approved <br /> changes in the scope, extent, or character of the project, the fee for CONTRACTOR's <br /> services shall be adjusted equitably through negotiation,based upon the unit prices listed <br /> in"Attachment B" and upon documented actual expenses for the additional work and <br /> materials. <br /> 5.0 INSURANCE <br /> 5.1 REQUIRED COVERAGES <br /> The CONTRACTOR shall procure and thereafter maintain and pay the premiums for <br /> insurance protection from claims including but not limited to the following: <br /> claims under worker's compensation acts; claims for damages resulting from <br /> personal injury accidents; claims for damages resulting from property damage <br /> accidents; and claims for damages resulting from professional errors and omissions. <br /> Insurance coverage shall be maintained to meet or exceed the following: <br /> A) Workers' Compensation Insurance endorsed with a Broad Form All States <br /> Coverage, which shall cover all the CONTRACTOR'S employees and <br /> subcontractors engaged in the performance of the work. <br /> B) Comprehensive General Liability Insurance Coverage , including broad <br /> form contractual liability coverage, with limits of at least One Million <br /> Dollars ($1,000,000) per occurrence, bodily injury and property damage. <br /> Decatur Public Transit System <br /> Underg�-ound Tank Monito�-ing Upgrades � <br />
