CHANGE ORDER Order No. 11 Date: 4/29/2002 �' Agreement Date: 4/24/2001 NAME OF PROJECT: SWTP Improvements Nitrate Removal Facility City Project No. 9953 OVYNER: City of Decatur, Illinois CONTRACTOR: Diversified Buiidings, inc. The following changes are hereby made to the CONTRACT DOCUMENTS: The change order wiil modify the contract as foAows: A. Remote Contro/ Camera. Furnish and install a remote control camera (Pelco Spectra II Series Discreet Dome System, Pendant Mount, Environmental service Model DD56C), a pendant for mounting the camera on a parapet wall at the southwest comer of the new Nitrate Facility, and the outdoor power supply. The camera shall be connected to the existing plant CCN system controller and multiplexer using RG-6/U cable. 7he cable shall be routed in a spare conduit from the Nitrate Facifity to the Operations Building Main Electric Room then in new '/." GRC conduit from the electric room to the operations room. Attachments: D81's PCO No. 13 (includes quotes and catalog information on the camera and accessories). B. PVC Ridgid Sfee/ Condtrit Install PVC coated rigid steel conduit in lieu of the specified galvanized rigid conduit for the new power and control circuits in the existing chemical storage and chemical feed areas of the Operations building. The routing and size of conduits shall be as indicated on the attached revised drawing sheets H12 and H13. Attachments:D81's PCO No. 14 (includes Modem Electric RFP-18 and RFP-19); Modem Electric Letter L-015 (includes Modem Electric RFP-18A); and revised Drawing Sheets H12 and H13. C. Nftrate Analyrers. !n lieu of the three specified UV-6101 units, fumish and install one UN-3150 single sample unit with crossflow filter, fitter cleaning system, and raw water sampling pump; one UV- 3150 and one UV-6101 unit both with eight sample units, for IX facilities. Feed Pump Pr+essure Switches and Operator ControUPLC Input. Fumish and install pressure switches on the suction side of the IX feed pumps. The pressure switches will protect the feed pumps f�om dange�ous operating head conditions by interlocking with the AFDs. Modify the lon Exchange Feed Pump Control description. Attachments: DBI's PCO No. 20(includes quotes) and 8&V RFP No. 6 dated January 9, 2002(includes Figures 1, 2, and 3)as mod�ed. E. F/owmeter and Vau1L Furnish and install a flowmeter vault, tapping saddles, air release valve and a multi-sensor inserition magnetic flowmeter. Complete MV transfoRners in preparation for the future generator. Attachments: DBl's PCO No. 23{includes quotes).
