Anderson Plaza <br /> Decatur, Illinois 62523-1196 <br /> Dear Mr. Meachum: <br /> This Letter Permit is your authorization to install a manhole and replace an <br /> existing catch basin in the 200 block of North Franklin Street (Business <br /> Route 51) east of Central Park in Decatur. <br /> This Letter Permit is being granted to the subject provisions: <br /> 1. Traffic Control for the required lane closure shall be in accordance with <br /> Highway Standards 701601 and 702001. <br /> 2. There shall be no additions or changes of location in the work authorized <br /> by this permit without prior approval of the District Engineer or his duly <br /> authorized representative. If deviations are made in the work authorized <br /> by this permit without approval, the Applicant may be required to remove <br /> the encroachments from right-of-way or to relocate the encroachments off <br /> the right-of-way at the Applicant's expense. <br /> 3. The Applicant shall locate, furnish, construct, and maintain the entire <br /> improvement at their expense and shall assume all risk or liability of every <br /> nature accruing during construction or thereafter as a result of the <br /> improvement. <br /> 4. The Applicant or their Contractor shall be required to perform their work in <br /> accordance with the "Standard Specifications for Road and Bridge <br /> Construction," and the "Supplemental Specifications" in effect on the date <br /> of construction. All material incorporated into the project shall be <br /> obtained from a Department approved source and shall be in accordance <br /> with Section 1000 of said "Specifications." <br /> 5. All underground facilities shall be located prior to construction. Call <br /> J.U.L.I.E. at telephone number 1-800-892-0123 at least 48 hours in <br /> advance. <br />
