Any underground utility or drainage facility, whether farm tile, sanitary , <br /> sewer, or storm sewer cut or damaged by this construction shall be � <br /> restored to functional condition and to the satisfaction of the owner of the ' <br /> facility. Existing highway ditch drainage shall be maintained at all times. <br /> 7. Any damage to the pavement due to construction or maintenance of the <br /> work authorized by this permit shall be repaired to the satisfaction of the <br /> District Engineer or duly authorized representative, by the Applicant at <br /> their expense. <br /> 8. The Applicant or their Contractor shall keep the highway pavement clear <br /> of mud, gravel, sand, or any other construction debris at all times. <br /> 9. It must be understood that everyone working on this project shall <br /> indemnify and save harmless the State of Illinois, its officers, and <br /> employees from all suits, actions, or damages received or sustained by <br /> any person or property on account of or in consequence of any act of <br /> omission, neglect, or misconduct, by your organization or anyone working <br /> with your organization. <br /> 10. The Applicant, his successors and assigns, agrees to hold harmless the <br /> State of Illinois and its duly appointed agents and employees against any <br /> action for personal injury, property damage, or claims arising out of <br /> environmental laws or regulations sustained by reason of the exercise of <br /> this permit. <br /> 11. One-way traffic is to be maintained at all times on Business Route 51 <br /> during this construction. <br /> 12. Class "SI" concrete shall be used in all proposed concrete construction on <br /> the right-of-way. <br /> 13. A curing period of 3 days shall be maintained for all concrete in the <br /> proposed entrance and curbing if high-early-strength cement or rich-mix <br /> (7 bag) concrete is used. If regular concrete is used, a curing period of 7 <br /> days is required. The surFace of the new concrete shall be kept moist and <br /> a minimum temperature of 40°F. shall be maintained during the entire <br /> curing period. <br /> 14. All cuts in existing highway pavement, paved shoulders, driveways, <br /> gutters, or curb and gutter shall be saw cuts to attain a vertical joint. <br /> 15. Proposed permanent patches in the existing highway pavement shall <br /> extend eighteen inches (18") beyond each side of the trench and shall be <br /> of material specified in the permit.
