Anderson Plaza <br /> Decatur, Illinois 62523-1196 <br /> Attention: Mr. William A. Ratekin <br /> Assistant City Engineer <br /> Dear Mr. Ratekin: <br /> This letter is your authorization to close a lane on Business Route 51 at <br /> the location depicted on the attached in order to facilitate point repairs <br /> to existing City sanitary sewers. I <br /> This Letter Permit is being granted subject to the following provisions: <br /> 1. Traffic control for the proposed lane closures shall be provided in <br /> accordance with attached Highway Standard Numbers 701601-03 <br /> and 702001-01. It is understood the lane closures shall only be <br /> during daylight hours, and all lanes will be opened to traffic at night. <br /> 2. Any damage to the pavement or to any underground drainage <br /> facility resulting from the work authorized by this Letter Permit shall <br /> be repaired to the satisfaction of the District Engineer or his duly <br /> authorized representative at the City's expense. <br /> 3. No deviations to the work specified in this Letter Permit will be <br /> allowed without prior approval from this office. <br /> 4. It shall be understood that everyone working on the project shall <br /> indemnify and save harmless the State of Illinois, its officers, and <br /> employees from all suits, actions, or damages received or <br /> sustained by any person or property or account of or in <br /> consequence of any act of omission, neglect, or misconduct by the <br /> City, their contractor, or anyone working with the City. <br />
