PLAN SELECTION PERIOD <br /> If your collective bargaining agreement provides for an annual Plan Selection Period every October,eligible <br /> Employees will be able to change benetit decisions based on which benefits and coverages are right for them and <br /> their Dependents if applicable. Benefit choices shall be either the Preferred Provider Organization(PPO)or the <br /> Comprehensive Major Medical Plan(CMM). <br /> Benefit choices made during the Plan Selection Period will become effective November lst and remain in effect until I� <br /> [he next November ls[.
