Contract Form C (5/00) Contract # <br /> Fiscal Year 2001 Appropriation #063-48250-1900-0000 <br /> CFDA# 14.900 <br /> STATE OF ILLINOIS <br /> DEPARTMENT OF PUBLIC HEALTH <br /> Grant Aqreement <br /> The Illinois Department of Public Heaith or its successor, hereinafter referred to as the "Department," and the City <br /> of Decatur, One Gary K. Anderson Plaza, Decatur, IL 62523-1196 hereinafter referred to as the "Grantee," hereby <br /> agree as follows: <br /> 1 Services: <br /> 1.1 The Grantee will provide the following services and agrees to act in compliance with all state and federal <br /> statutes and administrative rules applicable to the provision of services pursuant to this grant agreement. <br /> 1.1.1 The Grantee, in cooperation with the Macon County Health Department, will provide lead-based paint <br /> hazard reduction services to eligible clients in a program funded by the U. S. Department of Housing <br /> and Urban Development to evaluate and reduce lead-based paint hazards in an estimated 7 housing <br /> units. The Grantee will ensure that it will provide cost-effective services under this agreement by <br /> coordinating, integrating the lead-based paint hazard identification and clearance testing services of <br /> the Macon County Health Department. <br /> 1.1.2 The Grantee will accept and review applications from clients referred from the Department or its <br /> agent,the Macon County Health Department,to determine client eligibility.These referred clients shall <br /> have priority over other eligible clients. The property must contain lead-based paint hazards, be <br /> constructed before 1978, a 4-unit structure or less, with each unit having at least one bedroom and <br /> meet other conditions as specified in the Get the Lead Out Protocol. <br /> 1.1.3 The Grantee will assure that at least 50 percent of the rental units assisted be occupied by or made <br /> available to families with incomes at or below 50 percent of the area median income level and the <br /> remaining units shall be occupied or made available to families with incomes at or below 80 percent of <br /> the area median income level. The Grantee shall assure through lien or written agreement with the <br /> landlord that the landlord shall give priority in renting units assisted under this section, for not less than <br /> 3 years following the completion of lead abatement activities, to families with a child under the age of <br /> six years old. <br /> 1.1.4 The Grantee will assure that clients that are owner-occupants are the principal residents of the unit have <br /> an income at or below 80 percent of the area median income level. At least 90 percent of the owner- <br /> occupied units assisted shall be occupied by a child under the age of six years or shall be units where <br /> a child under the age of six years spends a significant amount of time visiting. <br /> 1.1.5 The Grantee will complete a detailed lead mitigation plan/work order in cooperation with the Macon <br /> County Health Department. This plan will list details of all the required worker and occupant safety <br /> measures, occupant relocation requirements, and all lead mitigation/abatement procedures and <br /> materials to be used. The Grantee will specify procedures and materials for each project as described <br /> in the current Get the Lead Oui Program protocol using the Department's work order generation <br /> � software program or equivalent method approved by the Department. The work order must be <br /> approved by the Macon County Health Department before work may begin. The Grantee will attempt <br /> to combine other housing rehabilitation work with lead hazard reduction work where possible. <br /> Page 1 of 6 I <br />
