. <br /> i ` � ' :' ' . <br /> ' �� ��� 3 <br /> � Z � <br /> u2 ' � <br /> HOME PROGRAM FY2001-02 � <br /> SUPPORTIVE HOUSING AGREEMENT <br /> � y�av <br /> THIS AGREEMENT, entered into on the � day of �� , 2001 by and between <br /> the City of Decatur, Illinois,hereinafter called the "City", and ove, Inc., organized under the <br /> laws of the State of Illinois. <br /> WHEREAS, the City has entered into an agreement with the U.S. Department of Housing and <br /> Urban Development,hereinafter called "HUD", for the purpose of conducting a HOME Program, <br /> and; whereas the City may commit funds for HOME funds for supportive housing projects; and <br /> whereas Dove Inc. submitted a proposal dated November 27, 2000 to conduct activities eligible <br /> under the HOME program guidelines and requested a grant of HOME funds not to exceed <br /> $63,000 to fund the activities under the stated proposal. <br /> NOW, THEREFORE, the parties in consideration thereto mutually agree and convenant as <br /> follows: <br /> I. SCOPE OF SERVICE I <br /> A. Dove Inc. may receive up to $63.000 from the City under the HOME Program to I�I <br /> implement the proposed activities. Funding will be dispersed based upon actual project , <br /> expenditures, for a total not to exceed the grant amount of$63,000. I <br /> B. Dove Inc. shall use the funds as outlined in the proposal dated November 27, 2000 for I <br /> eligible costs of predevelopment, acquisition, rehabilitation, construction, soft costs and <br /> other eligible items associated with the proj ects. All additional sources of funds for the <br /> project must be confirmed prior to the start of the project. <br /> C. Dove Inc. shall not obligate or expend any funds under the terms of this agreement <br /> until a written Notice to Proceed is issued by the Neighborhood Services Department - <br /> Neighborhood Renewal Division of the City. <br /> II. REPORTING AND REIMBURSEMENT PROCEDURE <br /> A. Progress Reports <br /> Dove Tnc. shall provide the City with quarterly progress reports and financial <br /> reports until the HOME program funds are expended. Quarterly reports shall be <br /> submitted to the City by the tenth (l Oth)working day of the succeeding calendar month. <br /> Quarterly reports shall include the status of the project, financial expenditures to date, <br /> any anticipated revisions to the project budget, and any other pertinent information. The <br /> City reserves the right to request additional information as necessary before processing <br /> reimbursement requests. Reimbursements will only be processed if Dove Inc. is current <br /> with submissions of quarterly reports. <br /> 1 <br />
