R2001-19 <br /> RESOLUTION AMENDING FLEXIBLE BENEFITS <br /> PLAN ADOPTION AGREEMENT <br /> SECTION 125 CAFETERIA PLANS <br /> BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF DECATUR, <br /> ILLINOIS: <br /> Section 1. That Article I, Definitions, I.16 of the City of Decatur's Flexible Benefits Plan <br /> Adoption Agreement, approved by the City Council of the City of Decatur, Illinois, on February <br /> 17, 1992, be, and is hereby, modified and amended so that, as modified and amended, said <br /> Article I, Definitions, I.16 shall read as follows, effective January 1, 2001: <br /> 1. Article I - Definitions, I.16 is amended to read "Change-in-status Events"means, and <br /> is limited to: <br /> a. change in legal marital status, including marriage, widowhood, divorce, legal <br /> separation, and annulment, <br /> b. increase or decrease in the number of dependents, including placement of a <br /> child for adoption, <br /> c. change in employrnent status, including a change in the individual's eligibility <br /> for an employee benefit plan, <br /> d. an individual's becoming or ceasing to be a dependent under the Plan's terms, <br /> and, <br /> e. change in place of residence, including a residence change that affects the <br /> accessibility of network providers. <br /> 2.
