,� , <br /> SUBRECIPIENT AGREEMENT <br /> THIS AGREEMENT, entered this 4th day of May , 2001 by and between the City of <br /> Decatur, Illinois (herein called the "City") and Decatur Not In Our Town (herein called the <br /> "Subrecipient"). <br /> WHEREAS, the City has applied for and received funds from the United States Government <br /> through the U.S. Department of Housing and Urban Development ("HLTD") under Title I of the <br /> Housing and Community Development Act of 1974, Public Law 93-383; and <br /> WHEREAS, the City wishes to engage the Subrecipient to assist the City in utilizing such funds <br /> in accordance with Federal regulations, HLJD guidelines, State and Local laws; <br /> NOW, THEREFORE, it is agreed that the application/proposal is a part of this agreement <br /> between the parties hereto that; <br /> I. SCOPE OF SERVICE <br /> A. Activities <br /> The subrecipient will be responsible for the Fiscal Yeaz 2001-2002 for providing <br /> education opportunities focusing on the elimination of racism. This program is <br /> designed to help children in grades 1-6 to make positive lifestyle choices and avoid <br /> unhealthy behaviors. Currently approximately 60 children are being served by the <br /> program,which meets once a week at a local church. I <br /> Program Delivery <br /> Activity: <br /> Benefit to low/moderate income families. <br /> General Administration <br /> Activity: <br /> B. Levels of Accomplishment-Performance Goals <br /> In addition to the normal administrative services required as part of this Agreement,the <br /> Subrecipient agrees to provide the following levels of program services: <br /> Activi Units per Month Total Units <br /> Kids with Character N/A 60 youth <br /> 1 <br />
