t � � ,. <br /> STATE REVOLVING FUND LOAN ASSISTANCE <br /> The Scope of Services for the City of Decatur Clarifier Conversion Project, State Revolving <br /> Fund Loan Assistance shall consist of the following tasks. <br /> 1. Project Plan. Prepare a project plan meeting the requirements for obtaining a State <br /> Revolving Fund (SRF) Loan. These requirements can be found at 35 III. Adm. Code <br /> Section 662.510. The Project Planning Submittal Checklist shall be completed as <br /> part of Project Planning. ENGINEER will assist CLIENT with the completion of the <br /> Pre-Application that is required to be submitted in conjunction with the Project Plan. <br /> 2. Preparation of Bidding 8� Contracting Documents. Specifications shall include <br /> the appropriate documents, forms, certifications and language as required for an <br /> SRF Loan project. Loan Applicant's Certification of Plans/Specifications Compliance <br /> with PWSLP Rules shall be completed. <br /> 3. IEPA Loan Applicant Environmental Checklist. Obtain the required <br /> environmental sign-offs for the project and complete the IEPA Loan Applicant <br /> Environmental Checklist for CLIENT signature. <br /> 4. PWSLP Loan Application Form. Assist CLIENT in the completion of the Loan <br /> Application Form including the accompanying Loan Program Certifications including: <br /> • Certification Regarding Debarment, suspension and Other Responsibility <br /> Matters. <br /> • Intent Regarding National Flood Insurance. <br /> • Certification Regarding Project Site, Rights-of-Way, Easements and Permits. <br /> • Authorization of a Representative to Sign Loan Documents. <br /> • Loan Applicant Acknowledgement of the Use of American Iron and Steel <br /> Products Requirement. <br /> • Taxpayer ldentification Number Form. <br /> 5. Financial Information Checklist. Assist CLIENT in the completion of the Financial <br /> Information Checklist. <br /> • CLIENT will provide the adequate documentation to meet the requirements of <br /> a Demonstration of a Dedicated Source of Revenue required for the loan. <br /> • ENGINEER will assist CLIENT with the development of a draft model <br /> ordinance, which authorizes the debt from the loan to be incurred. Ordinance <br /> to be finalized by CLIENT'S attorney or staff. <br /> • CLIENT will provide staff (attorney) to offer a legal opinion verifying the <br /> legality, enforceability and acceptability of the ordinance including coverage <br /> requirements (if any). <br /> A6 <br />
