� <br /> � h <br /> t I -��, <br /> AMENDMENT TO THE AGREEMENT <br /> This Amendment to the Agreement is made and entered into by and between the CITY OF <br /> DECATUR, ILLINOIS, a municipal corporation hereinafter referred to as the"City", and PRIDE <br /> MIDWEST SERVICES, INCORPORATED, d/b/a DECATUR PRIDE CHECKER CAB and <br /> d/b/a DECATUR YELLOW CHECKER CAB, an Illinois corporation authorized to do and <br /> doing business in the City of Decatur,hereinafter referred to as the"Cab Company". <br /> WHEREAS the City and the Cab Company entered into an Agreement, executed by the City on <br /> May 1, 2000, with an expiration date of October 31, 2000; and <br /> WHEREAS the Agreement between the City and the Cab Company lists the fares charged by the <br /> Cab Company, to be reimbursed by the City; and <br /> WHEREAS on October 1 st, the Cab Company raised their fares and requested reimbursement for <br /> the higher fares. <br /> NOW THEREFORE, for and in consideration of the promises, covenants, agreements and <br /> conditions hereinafter set forth to be performed, kept and met by the respective parties hereto, <br /> this Amendment to the Agreement is made by and between the CITY OF DECATUR, <br /> ILLINOIS, and PRIDE MIDWEST SERVICES, INCORPORATED, as follows: <br /> Subsection 3.C.1 of the Agreement shall be amended as follows: <br /> C. Reimbursement for Rides: <br /> 1. The fares charged Program participants by the Cab Company shall be made on the <br /> basis of the following rate schedule: $2.00 for the first one-tenth(1/lOth)mile; ten cents <br /> ($0.10) for each additional one-tenth (1/lOth)mile; thirty cents ($0.30)per minute <br /> waiting time; and $0.50 (fifty cents) for each additional passenger over six(6) years of <br /> age. <br /> - 1 - <br />
