8. Cab Company vehicles shall not wait for Program riders for more than five (�) <br /> minutes at any point. <br /> 9. The Cab Company shall account for all patrons transported,together with tl�e <br /> � appropriate charge, and shall maintain accurate trip sheets, Cab Ride Forms, and other , <br /> data which will provide sufficient information to permit evaluation of the Program by the I <br /> City, and by the State and Federal funding agencies. <br /> 10. The Cab Company shall permit the City to review or audit all records pertaining to <br /> this Proa am. <br /> 11. The Cab Company agrees that his Agreement is subject to the provisions of Exhibits <br /> A, B, and C attached hereto and made a part hereof. <br /> C. Reimbursement for Rides: <br /> 1. The fares chazged Program participants by the Cab Company shall be made on the <br /> basis of the existing rate schedule: $1.50 for the first one-tenth (1/l Oth) mile; ten cents <br /> ($0.10) for each additional one-tenth(1/lOth) mile; twenty cents ($0.20) per minute <br /> waiting time; and $0.50 (fifty cents) for each additional passenger over twelve (12) years <br /> of age: <br /> 2. Any change or alteration in the taxi-cab fare schedule shall require prior concurrence <br /> of the City. <br /> 3. The Cab Company shall bill the City at the end of each month for the cab fares <br /> charged for providing transportation services to Pro�ram participants. <br /> 4. The Cab Company shall submit a voucher for payment to the City at the end of each <br /> month. <br /> 5. Copies of all Cab Ride Forms and trip sheets shall be submitted to the City at the same <br /> time as the voucher for payment. <br /> 6. Cab Ride Forms must be completely filled out and signed by both the passenger and <br /> the driver before reimbursements will be made. <br /> 7. The City shall pay the voucher within 35 days after the voucher and the properly <br /> completed Cab Ride Forms and trip sheets aze received from the Cab Company. <br /> 4. The City shall have the responsibility for establishing the guidelines for the Projram. <br /> 5. The City shall register Program participants, determine eli�ibility, and furnish proper <br /> identification to eligible participants. <br /> � <br /> - � - <br />
