6 M . <br /> AGREEMENT <br /> This renewal AGREEMENT, made and entered into this first day of �,A �i , 2001 between <br /> CITY OF DECATUR, ILLINOIS MUNICIPALITY(hereinafter referred to as"CITY")and ST. MARY'S HOSPITAL, <br /> DECATUR, OF THE HOSPITAL SISTERS OF THE THIRD ORDER OF ST. FRANCIS,AN ILLINOIS NOT-FOR- <br /> PROFIT CORPORATION (hereinafter referred to as"HOSPITAL"), <br /> WITNESSETH: <br /> WHEREAS, the CITY is desirous of renewing its contract with the HOSPITAL,which shall continue to provide <br />, an Employee Assistance Program (hereinafter referred to as"EAP")through the Employee Service Center, and <br /> WHEREAS, the HOSPITAL'S EAP is desirous of continuing to provide assessment and referral services to <br /> employees and their dependents of CITY who are experiencing behavioral/health problems that have affected <br /> or could potentially impact their work performance, <br /> NOW THEREFORE, in consideration of the mutual covenants herein contained, the parties agree as follows: <br /> 1. Services to be Provided: <br /> The HOSPITAL'S EAP shall continue to perform the following services on behalf of the CITY: <br /> A. On-going consultation to the CITY on the internal design of the EAP,which shall include <br /> assistance in evaluating and maintaining the CITY'S EAP service. Quarterly review <br /> meetings shall be conducted with designated representative(s) of the CITY for the <br /> purpose of reviewing the effectiveness of the EAP and material promoting the EAP, e.g. <br /> articles for company newsletters, posters, brochures, payroll stuffers, and direct mail <br /> announcements sent to the homes of employees. <br /> B.
