. ti, . , ii <br /> C. Assessment and referral services for all CITY employees and their dependents through <br /> � � the EAP on a voluntary or supervisory referral basis. The assessment and referral <br /> components shall include: <br /> ❖ Problem assessment and diagnosis <br /> ❖ Resource identification and referral to appropriate helping agency <br /> ❖ Contact with referral agency to make sure employee arrived, services are <br /> appropriate, and employee is participating at a satisfactory level <br /> ❖ Brief counseling, up to six(6)one-hour sessions <br /> ❖ Feedback to the CITY'S EAP liaison or supervisor, stating level of employee <br /> participation, when referred by the CITY'S EAP liaison or supervisor <br /> D. Provide the following promotional materials: <br /> •:� Quarterly EAP Newsletters <br /> ❖ Informational pamphlets describing the EAP concept,for inclusion with the pay <br /> statements. <br /> ❖ One(1) EAP poster for each specified location <br /> E. A quarterly statistical report completed by the EAP, which shall include a statistical <br /> summary of all EAP services provided to the CITY during the reporting period. In <br /> addition, the EAP will provide a monthly report outlining the number of employees and <br /> families seen, and the number of hours spent on assessments, referrals, and <br /> supervisory consultations for the CITY'S EAP. <br /> F. One (1) training session, up to a maximum of three (3) hours, on the EAP for a <br /> maximum of twenty(20)CITY supervisors,when requested. This training will include <br /> the procedures and techniques in making a supervisory referral to the EAP. The EAP <br /> will provide a supervisory training manual specifically tailored to the policies and <br /> procedures of the CITY. <br /> G. Periodic supervisor/manager refresher training sessions, up to a maximum of two(2) <br /> hours each, on the activities of EAP, supervisory referral procedures, and <br /> St. Mary's Employee Service Center, 1900 E.
