Six(6)one(1) hour inservices or seminars related to the EAP service to the CITY each <br /> year,when requested. The EAP and CITY will meet to plan the design and delivery of <br /> the inservice program. The EAP will provide all materials necessary for the program. <br /> I. A periodic client survey is completed by the CITY and sent to all participating <br /> businesses. The survey elicits the clienYs responses to timeliness of appointments, <br /> helpfulness of counselor, confidentiality of services, thoroughness in explaining <br /> community resources, and follow-up contacts. The survey also elicits similar responses <br /> to the referred professional, agency, or other services. <br /> The CITY shall continue to perform the following services: <br /> A. Develop, in consultation with the CITY, an EAP policy statement to reflect the <br /> procedures for using the EAP. <br /> B. Provide a designated liaison to work with the HOSPITAL'S EAP. <br /> 2. Service Location: <br /> Assessment and referral services shall be conducted at the Employee Service Center Office <br /> located at 1900 E. Lake Shore Drive, Decatur, Illinois or at other designated locations, which <br /> I from time to time, ma be mutually agreed to by both parties. <br /> Y <br /> 3. Service Hours: <br /> The EAP shall provide day, evening, and weekend hours on a scheduled basis to assess <br /> employees of the CITY. <br /> 4. Professional Staff: <br /> The HOSPITAL shall provide professional staff experienced in behavioral/health sciences and <br /> in assessment and referral services to conduct employee assessments. <br /> 5.
