Pavment for Services: <br /> The provision of services as outlined in this AGREEMENT shall be based upon the following <br /> charges: <br /> SERVICE: CHARGE: <br /> � Consultation on Evaluating and $1920.00 <br /> Maintaining EAP Services, including <br /> the Reporting System <br /> (Sections 1-A and 1-E) <br /> � CITY Consultation to Supervisors $15.00/Consult <br /> (Section 1-B) <br /> � Assessment and Referral Services $75.00/HR(Maximum $440./Assessment) <br /> to Employee and Families <br /> (Section 1-C) <br /> � Promotional Materials $564.00 <br /> (Section 1-D) <br /> � Supervisor Training Session $165.00 <br /> (Section 1-F) <br /> � Supervisor Refresher Session $110.00 <br /> (Section 1-G) <br /> � EAP Inservices or Seminars $75.00/hour <br /> (Section 1-H) <br /> Charges for the Consultation on Evaluating and Maintaining EAP Service, including the <br /> Reporting System, totaling$1,920.00 will be payable at the beginning of each contract period. <br /> Charges for Promotional Materials totaling $564.00 will be divided into four quarterly <br /> St. Mary's Employee Service Center, 1900 E.
